Summary
A 50-year-old woman is seen in the office for the evaluation of postmenopausal bleeding. Pelvic ultrasound demonstrated an 11 mm endometrial lining. She has a history of diabetes and well-controlled hypertension. She has no known drug allergies. She has a history of prior cesarean sections. After review of technical aspects and risks, consent is obtained. She is placed in dorsal lithotomy position and the vagina is prepped with povidone-iodine. Vaginoscopy is performed using a 3 mm flexible hysteroscope. The vaginal mucosa and endocervical canal appear normal. She reports to the nurse that she is feeling lightheaded and warm. She subsequently states that her vision is blurred and loses consciousness.
Changes in macular pigment optical density and serum concentrations of lutein and zeaxanthin in response to weight loss

The objective of the present study was to investigate whether weight loss is associated with changes in serum concentrations of lutein (L) and zeaxanthin (Z), and/or macular pigment optical density (MPOD). We recruited 104 overweight subjects into this randomised controlled weight loss study. For the intervention group (I group), weight was assessed weekly and body composition, including BMI (kg/m2) and body fat (kg and percentage), was assessed at baseline, 6 and 12 months. Weight loss was encouraged using dietary and exercise programmes. MPOD was measured by heterochromatic flicker photometry and serum concentrations of L and Z by HPLC (at baseline, 1, 3, 6 and 12 months). The control (C) group was assessed at baseline and 12 months. Repeated-measures ANOVA (RMA) demonstrated significant weight loss in the I group over the study period (P = 0·000). There was no significant weight change in the C group (P = 0·993). RMA of dietary L and Z, serum L and Z, and MPOD demonstrated no significant time or time × group interaction effect in any of these parameters (P>0·05 for all), with the exception of a significant decrease in the dietary intake of Z seen in both groups, over the study period (P < 0·05). There was a positive and significant relationship between body fat loss (kg) and increase in serum concentrations of L in the I group (r 0·521; P = 0·006). Our finding that a reduction in body composition (e.g. fat mass) is related to increases in serum concentrations of L is consistent with the hypothesis that body fat acts as a reservoir for this carotenoid, and that weight loss can positively influence circulating carotenoid levels.

Summary
Introduction
Evidence mounts that humankind is changing the Earth's energy balance. The change in energy balance is attributable to the build-up in the atmosphere of greenhouse gases (GHGs) that partially trap outgoing long-wave radiation – that is, radiation given off by the Earth as a result of absorbing solar (short-wave) radiation. There is still some debate as to how much of the change in energy balance has shown up to date in the form of changes in climate-related variables such as global average temperature and precipitation–evaporation patterns. But there is overwhelming evidence that some GHG-induced change has occurred, as distinct from changes attributable to natural phenomena (solar or volcanic) or factors affecting long-term variability in the earth's climate (Solomon et al. 2007). We also know that at least some (perhaps half) of the imbalance is temporarily hidden – stored in the oceans (Hansen and Nazarenko 2005). Almost certainly as the twenty-first century progresses the climatological evidence of human-induced change will mount – and so will the impacts on the environment and vulnerable aspects of the economy and society.
There are ongoing attempts to frame a climate policy to succeed the Kyoto Protocol. Unless there is an epiphany in climate policy thinking, the emphasis will be on how much to do in the next period, rather than how to do it.
Systematic review of the role of gender as a health determinant of hospitalization for depression

Objectives: To conduct a systematic review of selected health determinants, including gender, and their impact on hospitalization rates for depression. Depression includes both depressive and bipolar disorders. Selected health determinants were gender, age, sex, family structure, education, and socioeconomic status.
Methods: Systematic search of conventional and fugitive literature sources. All reports of primary data, systematic reviews, and meta-analysis of primary data were included if they focused on hospitalization for depression and reported data by one or more of the selected health determinants. Two researchers independently evaluated each citation for inclusion and extracted data from the included studies.
Results: There is an important underreporting of health determinants data in studies of hospitalization for depression. No studies examined the role of gender. Age and sex were reported in 83 percent and 80 percent of the 110 included studies. Women showed a higher rate of hospitalization for depression than men (p<.05). Age and diagnosis had different effects in men and women. Adult women were significantly more likely than men to report a depressive disorder, whereas men were more likely to report a bipolar disorder (p<.05). Little can be concluded on the other health determinants.
Conclusions: The importance of reporting hospitalization data and conducting hospital utilization analysis by sex and health determinants, including gender, must be emphasized.
BEYOND MEDLINE: Reducing Bias Through Extended Systematic Review Search

Objective: To evaluate the sensitivity and precision of various extended search methods in identifying randomized controlled trials (RCTs) for systematic reviews.
Method: Prospective analysis of extended search methods (specialized databases or trial registries, reference lists, hand-searching, personal communication, and Internet) used in two systematic reviews of RCTs. The gold standard was the total number of RCTs identified by major databases (MEDLINE, EMBASE, etc.) and extended search strategies combined. Sensitivity was the proportion of all known RCTs identified by any extended search method. Precision reflected the proportion of all items uncovered by any extended search method that actually were RCTs.
Results: The extended search identified 94 additional RCTs for the systematic reviews beyond those identified with the major databases. Specialized databases and trial registries had the highest sensitivity and precision for the lipid-lowering project (13.6% and 52.7%, respectively; p < .05) followed by scanning of reference lists (7.2% sensitivity and 41.9% precision; p < .05). Hand-searching was more effective than personal communication and Internet searching (1.7% sensitivity and 12.2% precision; p < .05). The acupuncture project had slightly different results, with the specialized databases and trial registries tied with the review of reference lists for highest sensitivity (14.2%). The precision followed the same trend as the lipid-lowering project (17.6% specialized databases; 8.3% reference lists; p < .05). A post-hoc analysis showed that 75 of the 94 RCTs were indexed in the major databases but missed by the major database search.
Conclusions: Extended searching identified additional RCTs for the systematic reviews beyond those found in major databases. Specialized databases and trial registries were most effective. An important number of RCTs were missed by the major database search. Timing and accuracy of indexing may explain this finding. The definitive measure, whether there is an association between the method used to uncover RCTs, the quality of the items uncovered and their impact on systematic review results, is yet to be determined.
